Education of Young Children (NAEYC), The Elizabeth
Peabody House’s Preschool Program focuses on the
whole child. With a seasoned faculty, our program’s
holistic approach in and outside the classroom
is designed to stimulate the child intellectually,
physically, emotionally and socially. The curriculum
offers a host of learning opportunities, including
field trips, guest speakers and a low student-teacher
ratio. Another asset is its diverse student body whose
families hail from around the globe. With the world
getting ever-smaller in size and scope, our program
offers children a world view that increasingly is vital to
lifelong success.

Please note: The American Academy of Pediatrics has determined that placing a baby on his/her back to sleep reduces the risk of Sudden Infant Death Syndrome (SIDS). SIDS sit he sudden and unexplained death of a baby under one year of age. If you child does not usually sleep on his/her back, please contact your pediatrician immediately to discuss the best sleeping position for your baby. Please also take the time to discuss your child’s sleeping position with your caregiver.

I authorize staff in the child care program who are trained in the basics of first aid and CPR to give my child first aid and/or CPR when appropriate.
I understand that every effort will be made to contact me in the event of an emergency requiring medical attention for my child. However, if I cannot be reached, I hereby authorize the program to transport my child to the nearest medical care facility and to secure necessary medical treatment for my child.

Any “Other” transportation requests must be stated in writing and maintained in the child’s file or the above plan must be implemented. This permission is valid for one program year from the date of signature.
General Permissions
1. Daily Walking Field Trips: I give permission for my child to attend daily walking field trips as part of the EPH programs daily plan, including walks to Foss Park, Marshall Street Park, Somerville Public Library, Shaw’s and other neighborhood walks.
2. Photographs: I give permission for my child to be photographed by EPH staff for curriculum, classroom and agency use.
3. Consultation Services/Observations: I give permission for consultants from the Somerville Community Partnerships for Children to observe my child in the program as part of consultation services to EPH.
I understand that specific field trips and observations of my child for referral or special concern purposes will only be conducted with specific written permission for each activity.

Please note: As mandated by the Massachusetts Department of Early Education and Care, private pay families and those with a co-pay are billed for the 14 authorized closure dates for holidays and professional development. A calendar of these dates has been provided to you.
IMPORTANT INFORMATION

All Elizabeth Peabody House child care programs participate in the Child and Adult Care Food Program (CACFP), a Federal program that provides healthy meals and snacks to children receiving day care. EPH is reimbursed for serving nutritious meals which meet USDA requirements. It is very important that you fill out the attached paperwork. Even if you do not meet the requirements for free or reduced meals, we still need to file the information for all enrolled children. Your income and private information will not be shared with anyone or used for any purpose other than to be reimbursed for meals served to children at EPH.
